Event registration form template (fields, sheet and summary)
Two things in one file: the list of fields a registration form should ask, with type and whether it is required, and the sheet where registrations land, with ticket type, number of tickets, dietary needs, consent and status. Use it with Google Forms, a paper form, or as the blueprint for your own page.

No email address needed. Works in Excel, Numbers and Google Sheets (File, Import).
What is inside
- Form fields sheet
- Full name, email, phone, company, ticket type, number of tickets, dietary needs, how did you hear about us, consent to updates. Each with type and required flag.
- Registered on
- The date, so you can see how sign-ups build up towards the event.
- Ticket type and Tickets
- Standard, VIP, student, and how many. The summary sums the tickets.
- Status
- Confirmed, Waiting list or Cancelled. Counted on the summary sheet.
- Consent to updates
- Keep the yes or no next to the person, which is what GDPR asks for.
How to use it
- 1
Download the Excel file. Open the Form fields tab and remove the questions you do not need; fewer fields means more completed registrations.
- 2
Build the form with those fields in Google Forms, Typeform or on paper, or use the sheet itself at a registration desk.
- 3
Paste or type each registration into the Registrations tab. Set Status to Confirmed once you have replied, or Waiting list when you are full.
- 4
Watch the Summary tab for total registrations, tickets requested and the waiting list.
- 5
Before the event, sort by name and print, or export the list to a check-in sheet.
Where the spreadsheet stops
People register, then wait for a reply from you.
A GuestlistOnline registration page confirms them immediately with a QR ticket, or holds them for your approval if you prefer.
The form and the sheet fall out of sync, and capacity is a guess.
Registrations land on the guest list directly and the page closes itself when the limit is reached.
You need a payment step for tickets.
Connect Stripe and sell tickets with early bird and VIP tiers on the same page.
A registration page instead of a form and a sheet
Publish a branded registration page in minutes. Guests get a confirmation and QR ticket on the spot, you get the list, the capacity limit and the exports.
