RSVP form template: questions to ask and a sheet to track replies
An RSVP form only needs a handful of questions, and this template has them: attending yes or no, how many people, companion names, dietary needs, a message and the date of the reply. The summary sheet tells you how many replied, how many are coming and how many people that is in total.

What is inside
- Attending
- Yes or No. Keep it binary; "maybe" makes catering impossible.
- Number of guests
- Including the person replying. The summary sums it for everyone who said yes.
- Companion names
- So name tags, place cards and the door list are right.
- Dietary needs
- Free text, or restrict it to a few options if the caterer needs that.
- Message
- The nice part. Guests like to add a line.
- Responded on
- Who still needs a reminder.
How to use it

Download the file and keep the RSVPs tab as your master list.

Send your invitation with the questions from the columns: attending, number of guests, companion names, dietary needs, message.

Type each reply into a row, or link a Google Form to the sheet so replies appear on their own.

Check the Summary tab for total responses, attending, not attending and total people coming.

Two weeks before the event, sort by Responded on and remind everyone who has not answered.
Where the spreadsheet stops
Replies come in five different ways and you type them all.
A GuestlistOnline RSVP link puts a form in front of each guest; answers land on the guest list.
Reminders are manual and you lose track of who got one.
Automatic RSVP reminders go to guests who have not answered, in their language.
Guests need a ticket after they say yes.
Every confirmed guest receives a QR ticket by email, companions included.
